I. INFORMATION COLLECTED ABOUT YOU

A. Information Collected by Projectpcos.org or on Behalf of ProjectPCOS.org

Project PCOS collects both (1)Personal information, provided voluntarily by visitors through registrations, submissions or other methods and (2) Routine information and statistics gathered as visitors click through our website.

Your personal Information – Visitors may access and utilize our website without providing any personal information. However, there are services on the Project PCOS website which will require personal information through submission forms and/or registration to take advantage of the services we offer on our website. The personal information we may request from you may include your name, address, phone, e-mail and, if you are making donations out purchasing goods and services, your billing and credit card information. Providing your personal information to Project PCOS is strictly voluntary.

When would I be required to share my personal information? - Some situations and features in which we may request Personal Information include: Registration for our Forums and chats, newsletter registration, registration for fitness journals, personal story submissions, promotions, surveys, media story requests, research trial requests.

Sharing your information with third parities – Project PCOS may be required to share your personal information with third parties to fulfill services and requests. When processing donations, in some cases, we must share your Personal Information in order to fulfill your request(s). For example, we may use third party credit card processing companies and shipping fulfillment companies to fulfill your order. Additionally, from time to time we offer products or services on our Websites that are provided by a third party. Also, sometimes we offer contests and sweepstakes with another party. If you do not wish for us to share your Personal Information in this manner, please do not place orders on our Website or participate in contests or sweepstakes.

Registration & Member Profile.
In order to obtain access to certain services, and/or content available on our Website (e.g., fitness journals, chats, e-newsletter, forums), you must complete a registration form. Our Registration Form and Member Profile pages request that you submit the following information: your name, a selected username and password, email address, mailing address, zip code. From time to time we might contact you with regards to your one of your accounts, a billing question or to notify you regarding changes to our services.

You may not refuse receipt of any e-mails that deal with transactions. However, you are permitted to unsubscribe from all other e-mails that do not involve transactions made by you.

By registering for our services, you consent to giving Project PCOS the right to contact you regarding your account. Based upon the Personal Information you submit, you may receive welcome and/or verification e-mails for your accounts We may from time to time contact you via phone with regards to billing and/or submission questions and/or a request from you to provide information or answer questions. You may unsubscribe from receiving e-mails from us at any time.

Emails & Newsletters
You may register to receive email subscriptions and e- newsletters from Project PCOS. To receive emails and newsletters from Project PCOS you must provide your email address. You may cancel your subscriptions at any time.

Polls, Promotions, Sweepstakes
Project PCOS may offer you the opportunity to participate in contests, media requests, casting calls, sweepstakes, promotions or surveys on or through The Project PCOS Website. Participants may be required to share personal information in order to participate. Sharing of your personal information for these special promotions is strictly voluntary from you. Participation in these contests, sweepstakes, or surveys is completely voluntary. Contests may also be run in conjunction with a third party and therefore, when you are submitting your information, you are agreeing to share it with our partners, as well. The official rules of any promotion by, with or for Project PCOS, will be posted on the website. If a third party provider is being used, the provider will be prohibited from using your personal information for any other purposes.

Non-personally Identifiable Information
You may use our Website without registering or submitting any Personal Information. In that case, the only information we will collect from you is general, aggregated, demographic, or other non-personal information.

We currently use a third-party tracking service, to track non-personal information about visitors to our website in the aggregate. We may also share non-personal information with third parties such as advertisers and partners. This non-personal, aggregate data is used to analyze trends, to track user movement around the website, and to gather demographic information about how members, subscribers, and/or visitors use our website.

Children and Adolescents
We are committed to protecting the privacy of children. This Website is not designed for, nor is it intended to attract, children under the age of 18. We do not knowingly collect Personal Information directly from any person under the age of 18, and if we do inadvertently collect information from a person under the age of 18, we delete it as soon as we discover it.

Public Forums
This Privacy Policy does not protect you when you use the Project PCOS public forums. As a service to our users, Project PCOS features chat rooms and forums where users can share information and support with one another or post questions for experts to answer. In addition, we may from time to time offer online discussions on a variety of topics moderated by experts. You should be aware that any information shared in a chat room, bulletin board, or online discussion is public information and may be seen, disclosed to, or collected by third parties that do not adhere to our Privacy Policy. You should think carefully before disclosing any personal information in any public forum.

Emails to Project PCOS
This Privacy Policy does not protect you when you send business information, ideas, concepts or inventions to Project PCOS by email to the contacts listed on the "Contact Us" link on our Website. If you want to keep business information, ideas, concepts or inventions private or proprietary, do not send them in an email to Project PCOS.

II. DISCLOSURE

Except as set forth in this Privacy Policy or as specifically agreed to by you, Project PCOS will not disclose any Personal Information it gathers from you on our Website without your prior permission. Except as set forth herein, you will have the opportunity to opt-in to Project PCOS sharing your Personal Information with third parties, and you can withdraw your consent to Project PCOS sharing your information at any time. In some circumstances, we must disclose your Personal Information to third parties who help us administer our Website or our services. These third parties are required to keep your information confidential and do not use your information for any purpose other than the purpose under which your information was collected. Additionally, we offer the goods or services of third parties in our Online Store, and in other places on our Website. If you should make a purchase, we will have to disclose your information to these third parties so that your order can be placed and processed. I. Disclosure to Project PCOS Affiliates
We may disclose information (including Personal Information) about you to our Affiliates. For purposes of this Privacy Policy, "Affiliates" means any person or entity which directly or indirectly controls, is controlled by or is under common control with Projectpcos.org , whether by ownership or otherwise. Any information relating to you that we provide to our Affiliates will be treated by those Affiliates in accordance with the terms of this Privacy Policy.

II. Disclosure to Third Parties
Occasionally we may invite you to be contacted by third parties whose goods or services may be of interest to you. We will only share your Personal Information with these third parties if you grant us permission to do so. If you opt-in to such sharing, we may also share your Personal Information with licensors of certain health-related programs offered on or through our Website.

However, as a subscriber to the email newsletter, you are granting permission, in advance, for us to have a third party, health related advertiser to send an email to your account, but no more than 6 times in any calendar year.

We may provide third parties with non-personal information about you that does not allow you to be identified or contacted, including where such information is combined with similar information of other users of our Website. For example, we provide our advertisers and other third parties with aggregate, non-personal information about our Website users, including the number of unique users who visit our Website, the demographic breakdown of the registered users of our Website, or the activities that visitors to our Website engage in while on our Website.

III. Disclosure to Outside Contractors
We may employ independent contractors, vendors and suppliers (collectively, "Outside Contractors") to provide specific services and products related to our Website, including but not limited to maintaining an email suppression list as required by state and/or federal law; hosting and maintaining our chat rooms; providing credit card processing and shipping fulfillment; fraud screening; testing and implementation of special services such as those for contacting members to offer personal, one-on-one and/or member support to help members to reach their goals; and developing applications for our Website and email services. These Outside Contractors may sometimes have limited access to information collected on our Website, including your Personal Information, in the course of providing products or services to us. Access to your Personal Information by these Outside Contractors is limited to the information reasonably necessary in order for the Outside Contractors to perform their limited function for us.

Through our Website, you may be given the opportunity to purchase (or to obtain more information) regarding certain content, products or services which are sold through a person or company other than us or our Affiliates (collectively, "Third Party Sellers").

In addition, ProjectPCOS.org does not endorse and is not responsible for the privacy practices of these websites. VI. Other Disclosures
We reserve the right to transfer information to a third party in the event of a sale, liquidation, merger, or other transfer of all or substantially all of the assets of ProjectPCOS.org We will endeavor, in good faith, to gain the third party's agreement and adherence to the terms of this Privacy Policy. In addition, we will notify users of any such transfer through notice on our Website and through email.

The security of your Personal Information is important to us. We follow generally accepted industry standards to protect the Personal Information submitted to us, both during transmission and once we receive it. Except as otherwise stated herein, users of our Website are notified when Personal Information is being collected by third parties on our Website. You may always choose not to provide information, though it might be needed to take advantage of offers or Website features.

V. PERSONAL INFORMATION UPDATES

You can always contact us in order to (1) update or correct your Personal Information, (2) change your preferences with respect to communications and information you receive from us or third parties, or (3) delete the Personal Information maintained about you on our systems, by accessing your Member Profile and/or your Subscription Profile or by contacting Information@projectpcos.org . Such updates, corrections, changes and deletions will not have an effect on other information that we maintain, or information that we have provided to third parties in accordance with this Privacy Policy prior to such update, correction, change, or deletion.

You should be aware that it may not be possible to remove each and every record you have provided to ProjectPCOS.org from our servers. The need to back-up our systems to protect information from inadvertent loss means that a copy of your Personal Information may exist in a non-erasable form that will be difficult or impossible for us to locate. Nevertheless, we promise that upon receiving your request, all Personal Information stored in the databases we actively use for research and daily business activities, and other readily searchable media, will be disabled.

VI. PRIVACY-RELATED QUESTIONS

If you have a complaint or problem, you may email us at information@projectpcos.org and we will forward your complaint to the appropriate internal Project PCOS department for a response or resolution. We try to answer every email in a timely manner but are not always able to do so.
